To complete your registration with the Indigo team we will request copies of the following documentation as part of the welcome pack:
- Current CV
- GMC Certificate
- Valid and in date enhanced CRB disclosure / DBS – if you don’t have one we can replace this in-house
- Photograph identification e.g. Passport
- Two referees of which one must be your most recent employer
- Two Passport Photographs for identification badge purposes
- Proof of up to date immunizations
- Completed medical questionnaire
