To complete your registration with the INDIGO team we will request copies of the following documentation as part of the welcome pack:
- Current CV
- GMC Certificate
- Valid and in date enhanced CRB disclosure / DBS
- Photograph identification e.g. Passport
- Up to date MDU certificate
- Certificate of Completion of Training (Joint Committee certificate or equivalent)
- Letter from Primary Care Trust confirming which performance list you are on
- Two referees of which one must be your most recent employer
- Two Passport Photographs for identification badge purposes
- Completed medical questionnaire
